Abstract
Background

Hidradenitis suppurativa (HS) is a chronic inflammatory dermatosis strongly associated with obesity and metabolic dysfunction. GLP-1 receptor agonists (GLP-1 RAs), such as semaglutide, promote weight loss and exhibit anti-inflammatory properties, but evidence regarding their effects in HS remains limited.

Objective

To evaluate the effects of 6 months of semaglutide treatment at doses >1mg/week on anthropometric, dermatologic, metabolic, inflammatory, and quality-of-life outcomes in patients with HS and obesity.

Methods

We conducted a prospective, open-label cohort study including 20 adults with HS and obesity. Anthropometric, morphofunctional, dermatologic (Hurley stage, DLQI, pain VAS), psychological (BDI), metabolic, and inflammatory parameters were assessed at baseline and after 6 months.

Results

After 6 months, body weight (−21.4kg), BMI, waist circumference, blood pressure, and preperitoneal and subcutaneous fat were significantly reduced (all p<0.05). Dermatologic outcomes improved markedly: Hurley stage decreased in all patients, DLQI improved (−8.8), pain VAS decreased (−4.1), and BDI improved (−12.9; all p<0.001). Metabolic and inflammatory markers also improved. After adjustment for weight change, improvements in DLQI, pain, depressive symptoms, Hurley stage, vitamin D, CRP, and cortisol remained significant.

Conclusions

Semaglutide treatment in patients with HS and obesity improved weight, body composition, metabolic and inflammatory profiles, and dermatologic and psychological status. Several clinical and inflammatory benefits persisted after adjustment for weight loss.

Antecedentes

La hidradenitis supurativa (HS) es una dermatosis inflamatoria crónica asociada con la obesidad y la disfunción metabólica. Los agonistas del receptor del péptido similar al glucagón tipo 1 (GLP-1 RA), como la semaglutida, favorecen la pérdida de peso y presentan propiedades antiinflamatorias, pero la evidencia sobre sus efectos en la HS sigue siendo limitada.

Objetivo

Evaluar los efectos de 6 meses de tratamiento con semaglutida a dosis superiores a 1mg/semana sobre parámetros antropométricos, dermatológicos, metabólicos, inflamatorios y de calidad de vida en pacientes con HS y obesidad.

Métodos

Se realizó un estudio de cohorte prospectivo, abierto, que incluyó a 20 adultos con HS y obesidad. Se evaluaron parámetros antropométricos, morfofuncionales, dermatológicos (estadio de Hurley, DLQI, EVA de dolor), psicológicos (BDI), metabólicos e inflamatorios al inicio y a los 6 meses.

Resultados

Tras 6 meses, el peso corporal (−21,4kg), el IMC, la circunferencia de cintura, la presión arterial y la grasa preperitoneal y subcutánea se redujeron significativamente (todos p<0,05). El estadio de Hurley disminuyó en todos los pacientes, el DLQI mejoró (−8,8), la EVA de dolor se redujo (−4,1) y el BDI mejoró (−12,9; todos p<0,001). Se observaron mejoras en los marcadores metabólicos e inflamatorios. Tras el ajuste por cambio de peso, las mejoras en DLQI, dolor, síntomas depresivos, estadio de Hurley, vitamina D, PCR y cortisol se mantuvieron significativas.

Conclusiones

La semaglutida en pacientes con HS y obesidad mejoró el peso, la composición corporal, los perfiles metabólico e inflamatorio, así como el estado dermatológico y psicológico. Tras ajustar por pérdida de peso, diversos beneficios clínicos e inflamatorios persistieron.
