Abstract
Introduction

Acute heart failure (AHF) is a leading cause of hospitalisation among older adults, associated with high mortality and frequent readmissions. Prealbumin has been proposed as a marker of nutritional status and systemic inflammation, while the PROFUND index stratifies long-term risk in multimorbid patients. However, the combined prognostic utility of both tools in AHF remains poorly defined.

Objective

To assess the individual and combined prognostic value of Prealbumin levels and the PROFUND index in predicting 12-month mortality and hospital readmissions in elderly patients admitted for AHF.

Methods

We conducted a retrospective, observational study using data from the National RICA Registry. Patients aged ≥65 years hospitalised for AHF with available Prealbumin levels and PROFUND index at admission were included. Associations with 12-month mortality and readmission rates were analysed using multivariate models, ROC curve analysis, and Kaplan-Meier survival estimates.

Results

A total of 609 patients were included (median age 80 years, 61% female). Low Prealbumin levels (<18 mg/dL) were observed in 51% of patients. One-year mortality was significantly higher in the low Prealbumin group (55.8% vs. 38.0%; p < 0.001). The PROFUND index showed a progressive association with mortality risk (p < 0.001). The combination of low Prealbumin and high PROFUND index improved the prediction of all-cause mortality (AUC = 0.685), but did not significantly impact readmission risk.

Conclusions

Both Prealbumin levels and the PROFUND index independently predict mortality in elderly patients hospitalised for AHF. Their combined assessment may enhance risk stratification, identifying vulnerable subgroups with higher short-term mortality. Their predictive value for hospital readmissions appears limited.

Introducción

La insuficiencia cardíaca aguda (ICA) representa una de las principales causas de hospitalización en adultos mayores, con alta mortalidad y riesgo de reingreso. La prealbúmina se ha propuesto como marcador del estado nutricional y de la respuesta inflamatoria, mientras que el índice PROFUND, permite estratificar el riesgo en pacientes pluripatológicos. La utilidad pronóstica combinada de ambos parámetros en ICA no ha sido suficientemente explorada.

Objetivo

Evaluar el valor pronóstico de la prealbúmina y del índice PROFUND, de forma individual y combinada, en relación con la mortalidad y los reingresos a 12 meses en pacientes mayores hospitalizados por ICA.

Métodos

Estudio observacional, retrospectivo, a partir de datos del Registro Nacional RICA. Se incluyeron pacientes ≥65 años hospitalizados por ICA, con determinación de prealbúmina e índice PROFUND al ingreso. Se analizaron las asociaciones con mortalidad y reingresos a 12 meses mediante análisis multivariante, curvas ROC y modelos de supervivencia de Kaplan-Meier.

Resultados

Se incluyeron 609 pacientes, mediana de edad 80 años, 61% mujeres. El 51% presentaron prealbúmina <18 mg/dL. La mortalidad a 12 meses fue significativamente mayor en el grupo con Prealbúmina baja (55,8% vs. 38,0%; p < 0,001). El índice PROFUND se asoció progresivamente con la mortalidad (p < 0,001). La combinación de Prealbúmina e índice PROFUND mejoró la capacidad predictiva de mortalidad global (AUC = 0,685), sin impacto significativo en los reingresos.

Conclusiones

La prealbúmina y el índice PROFUND son factores pronósticos independientes de mortalidad en ICA. Su uso conjunto podría mejorar la estratificación del riesgo, permitiendo identificar subgrupos de mayor vulnerabilidad. Sin embargo, su utilidad para predecir reingresos es limitada.
