Improving the quality of emergency care with a risk map

Abstract
Objective

To describe the process of developing a risk map in a hospital emergency department.

Method

A risk map was developed for the observation area of the emergency department of a secondary-level hospital belonging to the Regional Health Service of Andalusia. The work involved the systematic identification of potential problems, their prioritization, and the proposal of improvement measures, following a structured risk analysis methodology applied to patient safety.

Results

A total of 25 main problems were identified. After prioritization, the following critical areas were highlighted: direct patient monitoring; medication safety during treatment administration; standardization of clinical procedures; continuity of care during night shifts; appropriate management of patient isolation; analysis of previous initiatives; and the implementation and auditing of new improvement proposals.

Conclusions

The use of structured tools aimed at patient safety, such as risk maps, is both feasible and useful for identifying improvement areas in emergency services. Their application allows problems to be prioritized, interventions to be targeted, and a systematic framework to be established for the continuous improvement of healthcare quality.

Objetivo

Describir el proceso de elaboración de un mapa de riesgos en un área de urgencias hospitalarias.

Método

Se desarrolló un mapa de riesgos en el área de observación de urgencias de un hospital de segundo nivel de la Junta de Andalucía. El trabajo se llevó a cabo mediante la identificación sistemática de problemas potenciales, su priorización y la propuesta de medidas de mejora, siguiendo una metodología estructurada de análisis de riesgos aplicada a la seguridad clínica.

Resultados

Se identificaron 25 problemas principales. Tras su priorización, se destacaron como áreas críticas: la vigilancia directa de los pacientes; la seguridad farmacológica en la administración de tratamientos; la homogeneidad en los procedimientos asistenciales; la continuidad de la atención en horario nocturno; la adecuada gestión del aislamiento de pacientes; el análisis de iniciativas previas, y la implementación y auditoría de nuevas propuestas de mejora.

Conclusiones

El uso de herramientas estructuradas orientadas a la seguridad clínica, como el mapa de riesgos, resulta factible y útil para identificar áreas de mejora en los servicios de urgencias. Su aplicación permite priorizar problemas, orientar intervenciones y establecer un marco sistemático para la mejora continua de la calidad asistencial.
