Abstract

Objective To evaluate the impact of unilateral cochlear implantation (CI) on quality of life and audiological, functional and psycho-emotional outcomes in a cohort of individuals aged 60 years or older, and to identify predictors of these outcomes.

Methods A single-center, retrospective observational cohort study was conducted. The primary endpoint was the change in health-related quality of life, measured by the Health Utilities Index-Mark 3 (HUI-3). Audiological outcomes, quality of life (GBI, NCIQ, HISQUI-19), functionality (IADL, Barthel), and psycho-emotional status (GDS-15, DJGLS, DAS) were cross-sectionally assessed using validated instruments. Multivariate linear regression models were used to identify predictors.

Results From a target population of 278 individuals, a final sample of 71 was included (31.1% participation rate). The intervention yielded statistically significant and clinically relevant improvements in the primary endpoint of quality of life (mean HUI-3 gain: 0.14; p< 0.001) and in audiological outcomes (mean PTA gain: 63.6 dB; p< 0.001 and in intelligibility: 53%; p = 0.033). Regression analyses identified that higher perceived sound quality (HISQUI-19) was associated with higher HUI-3, higher apathy (DAS) with lower GBI, and greater functional independence (Barthel) with better PTA.

Conclusions Unilateral CI is highly effective in older adults, but the perceived benefit is strongly modulated by psycho-emotional factors and the subjective experience of sound, beyond objective audiological gain. Apathy and poor perceived sound quality emerge as key barriers to success. A nursing care model that systematically integrates psycho-emotional assessment and expectation management into the preoperative phase is needed to maximize the benefits of this technology.

Objetivo Evaluar el impacto del implante coclear (IC) unilateral en la calidad de vida y los resultados audiológicos, funcionales y psicoemocionales en una cohorte de personas implantadas con 60 años o más, e identificar los predictores de dichos resultados.

Método Se realizó un estudio observacional de cohorte retrospectiva en un único centro. El resultado primario fue el cambio en la calidad de vida medida con el Health Utilities Index Mark 3 (HUI-3). Se evaluaron de forma transversal mediante instrumentos validados los resultados audiológicos, la calidad de vida (Glasgow Benefit Inventory [GBI], Nijmegen Cochlear Implant Questionnaire [NCIQ], Hearing Implant Sound Quality Index [HISQUI-19]), la funcionalidad (Índice de Lawton y Brody [IADL], Barthel) y el estado psicoemocional (Geriatric Depression Scale (GDS-15), De Jong-Gierveld Loneliness Scale [DJGLS], Dimensional Apathy Scale [DAS]). Se utilizaron modelos de regresión lineal multivariable para identificar predictores de los resultados.

Resultados De una población diana de 278 personas, se incluyó en el análisis una muestra final de 71 (tasa de participación del 31,1%). La intervención produjo mejoras estadísticamente significativas y clínicamente relevantes en el resultado primario de calidad de vida (ganancia media en HUI-3: 0,14; p < 0,001) y en los resultados audiológicos (ganancia media en promedio de tonos puros [PTA]: 63,6  decibelios [dB]; p < 0,001 y en inteligibilidad: 53%; p = 0,033). Los análisis de regresión identificaron que una mejor calidad de sonido percibido (HISQUI-19) y un menor grado de apatía (DAS) y de dependencia funcional (Barthel) se asociaron con una mejor calidad de vida y con los beneficios percibidos.

Conclusiones El IC unilateral es altamente efectivo en adultos mayores, pero el beneficio percibido está altamente modulado por factores psicoemocionales y la experiencia subjetiva del sonido, más allá de la ganancia audiológica objetiva. La apatía y una baja calidad de sonido percibido emergen como barreras clave para el éxito. Se necesita un modelo de atención enfermera que integre sistemáticamente la valoración del estado psicoemocional y la gestión de expectativas en la fase preoperatoria para maximizar los beneficios de esta tecnología.
