Fundamento

Conocer el perfil de los pacientes con ictus consultados a la unidad de geriatría y la utilidad de la valoración geriátrica integral (VGI) en la derivación al alta

Método

Se utilizaron escalas de VGI aplicadas por enfermeras para conocer la situación funcional, mental y social antes y después del ictus y los pacientes se agruparon según el destino al alta

Resultados

Se revisaron 1.042 protocolos. La situación previa al ictus era buena. La derivación al alta fue: a) 405 (39%), que presentaban buena situación funcional y mental, fueron enviados al domicilio; b) 508 (49%), que presentaban incapacidad funcional grave y buena situación mental, fueron enviados a unidades de media estancia; c) 97 (9%), que presentaban peor situación funcional y mental, fueron enviados a una unidad de larga estancia, y d) 32 (3%) fueron enviados a una residencia de ancianos. Existieron diferencias significativas en las escalas de VGI entre estos subgrupos

Conclusiones

La VGI supone un método de trabajo útil para enfermería, que permite individualizar los cuidados, cuantificar incapacidades y proporcionar el nivel asistencial más adecuado a cada paciente

Background

To study the profile of patients with stroke referred to a geriatric unit and to assess the usefulness of comprehensive geriatric assessment (CGA) to decide discharge setting

Method

CGA scales were applied by nurses to determine functional, mental and social status before and after stroke. The patients were grouped according to discharge setting

Results

A total of 1042 protocols were reviewed. Status prior to stroke was good. Discharge setting was: a) 405 patients (39%) presenting the best functional and mental status were discharged to home; b) 508 patients (49%) with severe functional disability and good mental status were sent to medium-stay geriatric units; c) 97 patients (9%) presenting the worst functional and mental status were sent to long-stay units, and d) 32 patients (3%) were sent to a nursing homes. Significant differences were found among these subgroups in CGA

Conclusions

The CGA is a useful tool in nursing that can be used to assess patient impairment and to individualize care requirements at discharge
