Abstract
Introduction

Burkholderia cepacian complex (Bcc) is a group of bacterial species which are phenotypically diverse and ubiquitous in the environment. They are gram-negative bacilli and opportunistic pathogens, infecting patients that are immunosuppressed, hospitalized and/or diagnosed with cystic fibrosis. In October 2024, an outbreak of five patients with positive blood culture for Burkholderia stabilis was detected in the Intensive Care Unit of a terciary care hospital.

Objectives

The aim of this article is to describe the outbreak from its detection to the identification of the reservoir and its resolution.

Material and methods

A case was defined as any patient with a positive blood culture for Bcc, admitted in the Intensive Care Unit (UCI) for more than 48 h. Samples were collected from the patient's environment and of ultrasound gels (GU) to search for reservoirs. The outbreak was confirmed by genomic sequencing of strains isolated from positive samples at the National Centre for Microbiology (Instituto de Salud Carlos III).

Results

All cases required ultrasound scanning and guidance techniques for the insertion of central catheters. Blood cultures from the five patients and the samples of GUs in use and one batch in the warehouse, factory sealed, tested positive to Burkholderia stabilis ST-1565.

Conclusions

Once detected the contamination of the GUs, the contaminated batch was removed from both warehouses and clinical units. After the implementation of the control measures no new cases were detected.

Introducción

Burkholderia cepacia complex (Bcc) constituye un grupo de especies bacterianas fenotípicamente diversas y ubicuas en el ambiente. Son bacilos gramnegativos y patógenos oportunistas, infectando a pacientes inmunodeprimidos, hospitalizados y/o diagnosticados de fibrosis quística. En octubre de 2024 se detectó, en la Unidad de Cuidados Intensivos de un hospital de tercer nivel, un brote de 5 pacientes con hemocultivos positivos para Burkholderia stabilis.

Objetivos

El objetivo del artículo es describir el brote desde su detección hasta la identificación del reservorio y resolución.

Material y métodos

Se definió como caso a todo paciente con algún cultivo positivo para Bcc ingresado en la Unidad de Cuidados Intensivos (UCI) más de 48 horas. Se recogieron muestras del entorno del paciente y de geles de ultrasonido (GU) para buscar el reservorio. El brote se confirmó mediante secuenciación genómica de las cepas aisladas en las muestras positivas, en el Centro Nacional de Microbiología del Instituto de Salud Carlos III (CNM-ISCIII).

Resultados

Todos los casos habían requerido técnicas ecográficas de exploración y guiado para la inserción de catéteres centrales. Se aisló Burkholderia stabilis ST-1565 en los hemocultivos de 5 pacientes ingresados en la UCI y en muestras de GU en uso y un lote sin abrir del almacén.

Conclusiones

Al detectar la contaminación de los GU se procedió a la retirada del lote contaminado de los almacenes y unidades clínicas. Tras implementar las medidas de control no se detectaron nuevos casos.
