Abstract
Background

Sleeve gastrectomy (SG) has become the most frequently performed bariatric procedure worldwide, however a substantial proportion of patients ultimately require conversion surgery due to complications or suboptimal outcomes. Comprehensive data on the results of the various conversion procedures remain limited.

Objective

To evaluate the indications, surgical techniques, safety profile, comorbidity resolution, and effectiveness of conversion bariatric surgery after SG.

Methods

Retrospective analysis of 209 patients who underwent conversion surgery after SG at Bellvitge University Hospital between 2015 and 2024. Primary outcomes included weight-loss results, complication rates, and comorbidity resolution. Secondary outcomes included length of hospital stay and achievement of Textbook Outcome.

Results

Mean age was 48.1 ± 7.8 years. Median maximum BMI was 54.8 kg/m2 (IQR 47.7–60.6). Indications were weight regain (55.5%), insufficient weight loss (32.5%), and GERD (12.0%). Conversion procedures included DS (48.3%), SADI-S (40.7%), and RYGB (11.0%). Overall complication rate was 7.2% with no mortality. Median total weight loss from maximum weight was 36.5% (IQR 27.9–45.0) and from pre-conversion was 14.0% (IQR 6.6–23.1 %). Hypertension prevalence decreased from 40.2% post-SG to 15.8% post-conversion (60.7% improvement). Dyslipidemia improved from 24.4% to 7.2% (70.6% improvement). Type 2 diabetes decreased from 25.4% to 11.5% (54.7% improvement). Obstructive sleep apnea decreased from 47.4% to 17.2% (63.6% improvement).

Conclusion

Conversion surgery after SG is safe and effective, achieving excellent outcomes. Hypoabsorptive procedures showed superior metabolic results while maintaining acceptable complication rates.

Antecedentes

La gastrectomía vertical (GV) se ha convertido en el procedimiento bariátrico más frecuentemente realizado a nivel mundial, pero una proporción sustancial de pacientes acaba requiriendo cirugía de conversión debido a complicaciones o resultados subóptimos. Existen datos integrales limitados sobre los resultados de los diversos procedimientos de conversión.

Objetivo

Evaluar las indicaciones, técnicas quirúrgicas, perfil de seguridad, resolución de comorbilidades y efectividad de la cirugía bariátrica de conversión tras GV.

Métodos

Análisis retrospectivo de 209 pacientes con cirugía de conversión tras GV en el Hospital de Bellvitge entre 2015−2024. Los objetivos principales incluyeron resultados de pérdida ponderal, tasas de complicaciones y resolución de comorbilidades. Los objetivos secundarios incluyeron estancia hospitalaria y consecución de Textbook Outcome.

Resultados

La edad media fue de 48,1 ± 7,8 años. La mediana del IMC máximo fue de 54,8 kg/m2 (RIC 47,7–60,6). Las indicaciones fueron reganancia ponderal (55,5%), pérdida ponderal insuficiente (32,5%) y ERGE (12,0%). Los procedimientos de conversión incluyeron CD (48,3%), SADI-S (40,7%) y BGYR (11,0%). La tasa de complicaciones global fue del 7,2% sin mortalidad. La mediana de PPT desde el peso máximo fue del 36,5% (RIC 27,9–45,0) y desde pre-conversión del 14,0% (RIC 6,6–23,1%). La prevalencia de hipertensión disminuyó del 40,2% al 15,8% post-conversión (mejoría del 60,7%). La dislipidemia mejoró del 24,4% al 7,2% (mejoría del 70,6%). La diabetes tipo 2 disminuyó del 25,4% al 11,5% (mejoría del 54,7%). La apnea obstructiva del sueño disminuyó del 47,4% al 17,2% (mejoría del 63,6%).

Conclusión

La cirugía de conversión tras GV demuestra seguridad y efectividad, con excelentes resultados. Los procedimientos hipoabsortivos mostraron resultados metabólicos superiores manteniendo tasas de complicaciones aceptables.
