The effect of pre- and perioperative anxiety on cardiovascular parameters and intraocular pressure of patients awaiting phacoemulsification surgery

Abstract
Introduction and objectives

To evaluate pre- and perioperative anxiety in patients scheduled for cataract surgery and to assess its correlation with cardiovascular parameters and intraocular pressure (IOP).

Patients or materials and methods

Thirty cataract surgery patients were included. Exclusion criteria included previous ocular surgery, glaucoma, and ocular inflammation. Anxiety levels were assessed using a visual facial anxiety scale (VFAS). Systemic blood pressure (BP), heart rate (HR), and IOP were measured using a sphygmomanometer and an air-puff tonometer, with mean values calculated from three readings. VFAS, BP, HR, and IOP were recorded during the pre- and perioperative periods. A t-test compared parameters, and Pearson’s coefficient was used to analyze correlations.

Results

Patients reported mild to moderate anxiety during both the pre- and perioperative periods. Cardiovascular parameters and IOP showed no significant differences between the two periods. IOP correlated significantly with HR perioperatively (R = 0.62, P < 0.001) and with systolic BP preoperatively (R = 0.47, P = 0.009). Preoperatively, patients with moderate to high anxiety had higher IOP (17.06 ± 1.69 mmHg) than those with no to mild/moderate anxiety (14.74 ± 2.41 mmHg; P = 0.006).

Conclusions

Higher anxiety levels increase IOP preoperatively. Assessing BP, HR, and IOP, as well as alleviating anxiety may improve surgical outcomes and ensure stress-free procedures.

Introducción y objetivos

Evaluar la ansiedad pre- y perioperatoria en pacientes programados para cirugía de catarata y su correlacion con los parámetros cardiovasculares y la presión intraocular (PIO).

Pacientes y métodos

Se incluyeron treinta pacientes sometidos a cirugía de catarata. Los criterios de exclusión incluyeron cirugía ocular previa, glaucoma e inflamación ocular. Los niveles de ansiedad se evaluaron mediante una escala visual facial de ansiedad (Visual Facial Anxiety Scale, VFAS ). La presión arterial sistémica (PA), la frecuencia cardíaca (FC) y la PIO se midieron mediante un esfigmomanómetro y un tonómetro de aire, calculándose los valores medios a partir de tres mediciones. La VFAS, la PA, la FC y la PIO se registraron en los periodos preoperatorio y perioperatorio. La comparación de los parámetros se realizó mediante la prueba t y las correlaciones se analizaron utilizando el coeficiente de Pearson.

Resultados

Los pacientes presentaron niveles de ansiedad leves a moderados en los periodos preoperatorio y perioperatorio. Los parámetros cardiovasculares y la PIO no mostraron diferencias significativas entre ambos periodos. La PIO se correlacionó significativamente con la FC en el periodo perioperatorio (R = 0.62; P < 0.001) y con la presión arterial sistólica en el periodo preoperatorio (R = 0.47; P = 0.009). En el periodo preoperatorio, los pacientes con ansiedad moderada a alta presentaron valores de PIO significativamente más elevados (17.06 ± 1.69 mmHg) en comparacion con aquellos con ansiedad nula a leve/moderada (14.74 ± 2.41 mmHg; P = 0.006).

Conclusiones

Los niveles elevados de ansiedad incrementan la PIO en el periodo preoperatorio. La evaluación de la PA, la FC y la PIO, así como la reducción de la ansiedad, podrían mejorar los resultados quirúrgicos y garantizar procedimientos libres de estrés.
