Abstract

This article examines the delineation of professional competencies in visual health care in Spain and the European Union, comparing the continental European model with the British, USA and Canada models. In Spain and most European countries, the diagnosis and treatment of eye diseases is the exclusive responsibility of ophthalmologists, supported by extensive medical training and strict regulation. Optometrists play a complementary role, focused on detecting refractive anomalies and adapting optical aids, without legal authority for medical diagnosis or treatment prescription. The article warns of the risks of expanding optometrists’ competencies without appropriate legal and educational reform, as this could compromise patient safety. It emphasizes that Anglo-Saxon models are not directly applicable to the Spanish context due to structural differences. The article advocates for clear interprofessional collaboration, well-defined referral protocols, and regular legal updates, always prioritizing patient safety.

Este artículo analiza la delimitación de competencias profesionales en la atención visual en España y la Unión Europea, comparando el modelo continental con el británico y anglosajón. En los primeros, el diagnóstico y tratamiento de enfermedades oculares corresponde exclusivamente al oftalmólogo, respaldado por una formación médica extensa y regulación estricta. Los ópticos-optometristas desempeñan un papel complementario, centrado en la detección de anomalías refractivas y adaptación de ayudas ópticas, sin atribuciones para el diagnóstico médico ni la prescripción de tratamientos. La ampliación de competencias de los ópticos-optometristas sin una reforma legal y formativa adecuada podría comprometer la seguridad del paciente y la calidad asistencial. Se subraya que los modelos anglosajones no son trasladables al contexto español por diferencias estructurales en formación, regulación y organización sanitaria. Se aboga por una colaboración interprofesional clara, protocolos de derivación definidos y actualizaciones legales periódicas, evitando la confusión competencial y priorizando siempre la seguridad del paciente.
