Abstract
Background and objective

To evaluate the efficacy and safety at 12 months of PRESERFLO® implantation combined with 0.04% MMC and the interposition of DuraGen® in patients with moderate or severe glaucoma, and to analyse bleb morphology using anterior segment optical coherence tomography (AS-OCT).

Material and methods

Prospective study including 50 eyes that underwent PRESERFLO® implantation with 0.04% MMC and DuraGen®, either as a standalone procedure or combined with cataract surgery. Complete success was defined as achieving an intraocular pressure (IOP) between 5 and 18 mmHg without topical hypotensive treatment, qualified success when this range was achieved with treatment, and failure when these criteria were not met. Postoperative complications and bleb morphology were assessed using AS-OCT. In addition, a comparative subanalysis between standalone and combined surgery was performed.

Results

Mean preoperative IOP was 26.8 ± 7.1 mmHg and decreased to 13.1 ± 4.5 mmHg, representing a mean reduction of 47.9% (P < .001). Complete success was achieved in 42 eyes (84%), qualified success in 5 eyes (10%), and failure was observed in 3 eyes (6%). The most frequent complication was choroidal detachment (22%). No significant differences were observed between standalone and combined surgery regarding final IOP or surgical success rates. In successful cases, AS-OCT more frequently showed a suprascleral aqueous lake, whereas failed cases presented a flat bleb.

Conclusions

PRESERFLO® implantation combined with 0.04% MMC and DuraGen® was effective and safe for IOP control at 12 months. AS-OCT allows identification of morphological patterns associated with success and failure.

Antecedentes y objetivo

Evaluar la eficacia y seguridad a 12 meses de la implantación de PRESERFLO® asociado a MMC al 0,04% y a la interposición de DuraGen® en pacientes con glaucoma moderado o severo, y analizar la morfología de la ampolla de filtración mediante OCT-SA.

Material y métodos

Estudio prospectivo sobre 50 ojos, intervenidos mediante implantación de PRESERFLO® con MMC al 0,04% y DuraGen®, como procedimiento aislado o combinado con cirugía de catarata. Se definió éxito completo como alcanzar una PIO entre 5 y 18 mmHg sin tratamiento hipotensor tópico, éxito cualificado cuando se alcanzó dicho rango con tratamiento, y fracaso cuando no se cumplieron estos criterios. Se evaluaron las complicaciones postoperatorias y la morfología de la ampolla mediante OCT-SA. Además, se realizó un subanálisis comparativo entre cirugía aislada y combinada.

Resultados

La PIO media preoperatoria fue de 26,8 ± 7,1 mmHg y se redujo a 13,1 ± 4,5 mmHg, lo que supuso una disminución media del 47,9% (p < 0,001). El éxito completo se alcanzó en 42 ojos (84%), el éxito cualificado en 5 ojos (10%) y se consideró fracaso en 3 ojos (6%). La complicación más frecuente fue el desprendimiento coroideo (22%). No se observaron diferencias significativas entre cirugía aislada y combinada en la PIO final ni en las tasas de éxito. En los casos de éxito, la OCT-SA mostró con mayor frecuencia un lago de humor acuoso supraescleral, mientras que los de fracaso mostraron una ampolla plana.

Conclusiones

La implantación de PRESERFLO® asociado a MMC 0,04% y DuraGen® fue eficaz y segura para controlar la PIO a los 12 meses.
