Abstract
Background and objective

To calculate the cost per patient of ophthalmological care for patients referred from primary care under a telematic model (Quixote® project), compared with conventional face-to-face care at the Hospital Universitario de Jerez, within the Jerez, Sierra de Cádiz, and Northwest Coast Health Area (Spain).

Material and methods

Comparative analysis of direct costs of telematic ophthalmological care versus the traditional face-to-face model, alongside a retrospective observational evaluation of efficiency after implementation of the Quixote® project. Outcomes included accessibility (mean waiting time in days for ophthalmological care, cumulative primary care waiting list) and patient satisfaction.

Results

The direct cost of complete care using the telematic model was €22.47 per patient compared with €32.5 per patient for the face-to-face model. Among 4288 patients evaluated in the Quixote® project at HUJ, 2659 patients (62%) required referral for face-to-face hospital assessment, while 1629 (37.9%) were discharged. The mean waiting time for patients referred from primary care decreased from 89 days to 39 days (−56.8%; 95%CI, 53.8%–58.5%; P < .0001). The cumulative waiting list decreased from 4606 to 1684 patients (−64%; 95%CI, 61.7%–66.3%; P < .0001).

Conclusions

Implementation of the Quixote® tele-ophthalmology program is a cost-efficient strategy for delivering ophthalmological care. It improves patient throughput from primary care, reduces waiting lists, optimizes hospital resources, and facilitates early detection of relevant diseases.

Antecedentes y objetivo

Calcular los costes por paciente para la atención oftalmológica de los pacientes derivados desde Primaria de un modelo telemático (proyecto Quixote®) frente al modelo de atención presencial clásico en el Hospital Universitario de Jerez del Area Sanitaria de Jerez, Sierra de Cádiz y Costa Noroeste.

Material y métodos

Estudio comparativo de costes para la atención telemática de oftalmología frente a los costes del modelo clásico presencial y un análisis retrospectivo observacional de eficiencia en la atención a los pacientes desde la aplicación del proyecto Quixote® abordando los cambios en la accesibilidad y valorando la satisfacción de los pacientes.

Resultados

Los costes directos calculados para la atención completa con modelo telemático son de 22,47 euros por paciente frente al modelo clásico presencial de 32,5 euros por paciente. De un total de 4.288 pacientes evaluados e informados en el proyecto Quixote® HUJ se han derivado 2.659 pacientes (62%) para revisiones hospitalarias presenciales y se han dado de alta a 1.629 pacientes (37,9%). La demora media en días para la atención de pacientes derivados desde primaria ha pasado de 89 días de 39 días, una disminución de un 56,8% (IC 95% 53,8% a 58,5%) (p < 0,0001). La lista de espera acumulada de pacientes de primaria ha pasado de 4.606 pacientes a 1.684 pacientes, una disminución de un 64% (IC 95% 61,7% a 66,3%) (p < 0,0001).

Conclusiones

Se demuestra que la implantación del proyecto Quixote® de telemedicina se trata de una forma de trabajo coste-eficiente. Permite aumentar rendimiento salida de primeras visitas en oftalmología y llegar a eliminar lista de espera acumulada, optimizar recursos de atención hospitalaria y detectar precozmente importantes enfermedades.
