Abstract
Background and objective

Study the correlation between pupillary diameter and intraoperative complications in patients with pseudoexfoliation undergoing phacoemulsification.

Material and methods

Prospective, observational, and descriptive study. Data were collected from May 2024 to May 2025.

Results

Preoperative pupillary diameter <5.5 mm post dilation was significantly associated with the development of intraoperative complications (P = .002). Among patients with complications, the median pupil dilation was 4.99 mm, whereas in those without complications the median was 5.94 mm. Most complications occurred during the phacoemulsification phase (6; 66.7%), followed by capsulorhexis (2; 22.2%). Additionally, the presence of zonular weakness was significantly associated with the development of complications (P < .001).

Conclusions

Preoperative identification of a pupil diameter <5.5 mm post dilation may alert the surgeon to potential intraoperative findings such as zonular weakness and iris instability, which may lead to better surgical planning.

Antecedentes y objetivo

Determinar si el diámetro pupilar se correlaciona con la incidencia de complicaciones transquirúrgicas en pacientes con pseudoexfoliación sometidos a cirugía de catarata por facoemulsificación.

Material y métodos

Estudio prospectivo, observacional y descriptivo. El estudio recabó datos de mayo de 2024 a mayo de 2025.

Resultados

La dilatación pupilar preoperatoria <5,5 mm se asoció significativamente con el desarrollo de complicaciones transquirúrgicas (p = 0,002). En los pacientes con complicaciones, la mediana de dilatación pupilar fue de 4,99 mm; mientras que, en aquellos sin complicaciones, la mediana fue de 5,94 mm, la mayoría de las complicaciones se produjeron durante la fase de facoemulsificación (6; 66,7%), seguido de la capsulorrexis (2; 22,2%). Por otro lado, la presencia de debilidad zonular se asoció de forma significativa con el desarrollo de complicaciones (p < 0,001).

Conclusiones

La identificación preoperatoria de un diámetro pupilar <5,5 mm puede alertar al cirujano sobre la presencia intraoperatoria de alteraciones como debilidad zonular e inestabilidad del iris, lo que permite anticipar la complejidad quirúrgica y planificar estrategias intraoperatorias.
