Abstract
Background and objectives

To evaluate intraocular pressure (IOP) and the number of hypotensive medications in patients with mild-to-moderate appositional primary angle-closure glaucoma (PACG) after phacoemulsification surgery (phaco-only group) versus phacoemulsification combined with the implantation of two iStent® W inject trabecular stents (Glaukos Corporation, CA, USA) (phaco-iStent group).

Materials and methods

A retrospective cohort study including 28 eyes from 28 patients with appositional PACG, divided into two groups (phaco-only and phaco-iStent). The number of medications and IOP values were recorded, at baseline, as well as, at one month, six months, and twelve months postoperatively.

Results

Baseline and one-month postoperative mean IOP were similar in both groups (P > .05). The phaco-iStent group exhibited significantly lower IOP at six and twelve months postoperatively compared to the phaco-only group (P = .024 at six months and P = .018 at twelve months). The number of active agents was significantly lower at twelve months in the phaco-iStent group (P = ,01).

Conclusions

There is greater efficacy in lowering intraocular pressure over time in patients with appositional PACG if phacoemulsification is accompanied by trabecular surgery. This could be useful in patients in whom we suspect that lowering IOP with phacoemulsification alone will not be sufficient to control the disease.

Antecedentes y objetivos

Evaluar la presión intraocular (PIO) y el número de fármacos hipotensores en pacientes con glaucoma primario de ángulo cerrado (GPAC) aposicional en estadío leve-moderado tras la cirugía de facoemulsificación (grupo faco-solo) versus facoemulsificación asociada a la implantación de 2 stents trabeculares iStent® W inject (Glaukos, Corporation, CA, EE. UU.), grupo faco-istent.

Materiales y métodos

Estudio de cohortes retrospectivo en el que se incluyeron 28 ojos de 28 pacientes con GPAC aposicional divididos en dos grupos (faco-solo y faco-istent). Se determinó el número de fármacos y el valor de la PIO basal, al mes, a los 6 meses y a los 12 meses tras la cirugía.

Resultados

La PIO media basal y al mes de la cirugía fue similar en ambos grupos (P > ,05). El grupo faco-istent presentaba una PIO significativamente más baja a los 6 y 12 meses de la cirugía respecto al grupo faco-solo (P = ,024 a los 6 meses y P = ,018 a los 12 meses). El número de principios activos fue significativamente menor a los 12 meses en el grupo faco-istent (P = ,01).

Conclusiones

Existe una mayor eficacia en la bajada tensional y la disminución del número de fármacos con el tiempo en los pacientes con GPAC aposicional si la facoemulsificación se acompaña de cirugía trabecular. Esto podría ser útil en pacientes en los que sospechemos que la bajada de la PIO con la facoemulsificación aislada no va a ser suficiente para controlar la enfermedad.
