Extramedullary intrathoracic hematopoiesis occurs in several hematological diseases and on chest films appears as a mass in the posterior mediastinum. The diagnosis may pose problems in differentiation from other processes also presenting masses in the posterior mediastinum. A diagnosis of intrathoracic hematopoiesis may be suspected when there are:

1. ses with well defined margins, which may be lobular, in the paravertebral thoracic region without signs of erosion or lesions in the adjacent bony structures.

2. lenomegaly or a history of previous splenectomy.

3. onic hematological disease, particularly congenital hemolytic anemia.
