Resumen

Se revisan las dificultades diagnósticas del cuerpo extraño endobronquial en el adulto, cuando el enfermo no relaciona el accidente aspirativo con su sintomatología broncopulmonar. Así ocurrió en 3 enfermos ingresados en nuestro Servicio en los 2 últimos años. En los 3 casos el diagnóstico fue causal, en 2 de ellos por broncofibroscopia realizada por presentar expectoración hemoptoica, y en el tercero por expulsión espontánea con la tos.

Ninguno de los enfermos había olvidado el accidente aspirativo, pero no lo relacionaron con su patología actual. La extracción se realizó en uno de los casos con la pinza de biopsia del broncofibroscopio, en otro se realizó broncoscopia con anestesia general, y en el último espontáneamente con la tos. Se resalta que el broncofíbroscopio, ha comenzado a desplazar al broncoscopio de la última de sus indicaciones, en el adulto hasta hoy incuestionable.

Summary

The authors review the diagnostic difficulties for and endobronchial foreign body in the adult when the patient does not relate the aspirative accident with his bronchopulmonary sy mptomatology. This is what happened to three patients admitted to the authors’ department during the last two years. In the three cases the diagnosis was casual; in two bases by bronchofibroscopy carried out due to the presence of hemoptysic expectoration, and in the third by spontaneous expulsión with the cough. None of the patients had forgotten the aspirative accident, but did not relate it to their current pathology. In one of the cases extraction was made with the biopsy clamp of the bronchofibroscopy; one case required bronchoscopy with general anesthesia; and the third was spontaneous with the cough. The authors emphasize the fact that bronchofibroscopy has begun to displace bronchoscopy from the last of its indications in the adult, which had been unquestionable until now.
