The worldwide incidence of tuberculosis (TB) has been increasing. Although its diagnosis is well established in adults, in children it is difficult due to its particular aspects. We report a 3 years and 8 month-old infant who experienced chronic wheezing, classified as moderate-to-severe asthma, had recurrent pneumonia, and was no responsive to the management with beta adrenergic agents. Chest X-rays (CXR) showed heterogeneous condensation in medium lobe and the chest computerized-tomography scan (CCT) a heterogeneous increase in pulmonary transparency, like condensation in the same lobe. After four months of treatment with anti tuberculosis agents, a significant improvement in symptoms, normal CXR, absence of pulmonary medium lobe condensation, and persistence fibro-atelectatic band in lingula were observed.

La incidencia de la tuberculosis en todo el mundo sigue aumentando. Aunque su diagnóstico está establecido en adultos, en niños es difícil por sus particulares aspectos. Se presenta el caso de un niño de 3 años y 8 meses de edad con dificultad respiratoria crónica clasificada como asma moderada-grave, con neumonía recurrente, que no respondía al tratamiento con beta-adrenérgicos. La radiografía de tórax mostró una condensación heterogénea en el lóbulo medio y en la tomografía computerizada de tórax se observó un aumento heterogéneo de la transparencia pulmonar, como una condensación en el mismo lóbulo. Los síntomas mejoraron significativamente tras cuatro meses de tratamiento antituberculoso, normalización de la imagen radiológica por desaparición de la condensación del lóbulo medio, aunque persistía una banda fibro-atelectásica en la lingula.
