Abstract
Introduction

Perioperative blood transfusion (BT) has been suggested as a potential prognostic factor in patients undergoing radical cystectomy (RC) for bladder cancer, although its impact remains controversial. We aimed to evaluate the association between perioperative BT and short- and long-term outcomes in a contemporary single-center RC series.

Materials and methods

We retrospectively analyzed consecutive patients who underwent RC for bladder cancer at our institution between 2017 and 2024. Demographic, perioperative, and pathological variables were collected. Outcomes included 30-day mortality, overall survival (OS), and cancer-specific survival (CSS). Survival analyses were performed using Kaplan–Meier estimates and compared with the log-rank test. Multivariable Cox and logistic regression models were constructed to adjust for potential confounders.

Results

A total of 200 patients were included (median age 69 years [31–88]), of whom 105 (52.5%) received perioperative blood transfusion (BT). Median estimated blood loss was 700 mL (100–3500). Thirty-day mortality was 6% (12 patients). Overall survival (OS) was 63.5% (127 patients), and cancer-specific survival (CSS) was 72.5% (145 patients) at the end of follow-up. Perioperative BT was significantly associated with worse overall survival (HR 3.28, 95% CI 1.88–5.72, p < 0.001), cancer-specific survival (HR 5.33, 95% CI 2.55–11.13, p < 0.001), and increased 30-day postoperative mortality (OR 10.88, 95% CI 1.38–85.6, p = 0.024). In multivariable analyses, perioperative BT remained independently associated with worse OS and CSS, together with advanced pathological T stage, positive nodal status, and higher ASA score, but was not independently associated with 30-day mortality.

Conclusions

Perioperative blood transfusion was associated with worse overall and cancer-specific survival after radical cystectomy, but not with early postoperative mortality after adjustment. These findings should be interpreted as an association rather than a causal relationship, as residual confounding cannot be excluded. Strategies aimed at optimizing preoperative anaemia and reducing transfusion requirements may contribute to improving perioperative management in patients undergoing radical cystectomy.

Introducción

La transfusión sanguínea perioperatoria (TS) se ha propuesto como un posible factor pronóstico en pacientes sometidos a cistectomía radical (CR) por tumor vesical, aunque su impacto continúa siendo controvertido. El objetivo de este estudio fue evaluar la asociación entre la TS perioperatoria y los resultados a corto y largo plazo en una serie contemporánea de CR de un único centro.

Material y métodos

Se realizó un análisis retrospectivo de pacientes consecutivos intervenidos de CR por tumor vesical en nuestro centro entre 2017 y 2024. Se recogieron variables demográficas, perioperatorias y patológicas. Las variables recogidas fueron la mortalidad a 30 días, la supervivencia global (SG) y la supervivencia cáncer específica (SCE). Los análisis de supervivencia se realizaron mediante estimaciones de Kaplan–Meier y se compararon mediante la prueba de log-rank. Se construyeron modelos multivariantes de regresión de Cox y de regresión logística para ajustar posibles factores de confusión.

Resultados

Se incluyó un total de 200 pacientes (mediana de edad 69 años [31–88]), de los cuales 105 (52,5%) recibieron transfusión sanguínea perioperatoria (TS). La mediana de sangrado estimado fue de 700 mL (100–3500). La mortalidad a 30 días fue del 6% (12 pacientes). La supervivencia global (SG) fue del 63,5% (127 pacientes) y la supervivencia cáncer específica (SCE) del 72,5% (145 pacientes) al final del seguimiento. La TS perioperatoria se asoció significativamente con peor SG (HR 3,28; IC 95% 1,88–5,72; p < 0,001), peor SCE (HR 5,33; IC 95% 2,55–11,13; p < 0,001) y mayor mortalidad postoperatoria a 30 días (OR 10,88; IC 95% 1,38–85,6; p = 0,024). En los análisis multivariantes, la TS perioperatoria se asoció de forma independiente con menor SG y SCE, junto con estadio patológico T avanzado, presencia de afectación ganglionar y mayor clasificación ASA, aunque no se asoció de forma independiente con la mortalidad a 30 días.

Conclusiones

La transfusión sanguínea perioperatoria se asoció con una menor supervivencia global y cáncer específica tras la cistectomía radical, pero no con una mayor mortalidad postoperatoria temprana tras el ajuste multivariante. Estos hallazgos deben interpretarse como una asociación y no como una relación causal, ya que no puede excluirse la confusión residual. Las estrategias dirigidas a optimizar la anemia preoperatoria y a un uso transfusional restrictivo podrían contribuir a mejorar el manejo perioperatorio de los pacientes sometidos a cistectomía radical.
