Abstract
Introduction and objectives

Benign prostatic enlargement has a prevalence of approximately 60% in 60-year-old men. En bloc enucleation and its variants, which emerged in 2010, are methods to enucleate the three lobes of the prostatic capsule completely, maintaining the capsular plane and the urethral sphincter while removing the prostate. The aim of the present study is to analyze the implementation of the surgical technique, considering the learning curve and the experience accumulated in a group of patients before and after its development.

Material and methods

Retrospective study from July 2022 to November 2024, in which 136 patients with lower urinary tract symptoms due to prostate growth with indication for surgical treatment were included. Bleeding, surgical time, hospital stay, enucleated prostate volume, as well as enucleation, morcellation, time and laser energy efficiencies were analyzed.

Results

The efficacy of the procedure was higher in the post-curve group with a p < 0.0001 compared to the pre-curve group with a coefficient of determination R2 = 0.8091. Enucleation efficiencies, laser time and energy showed a statistically significant difference (p < 0.05) in favor of the post-curve group.

Conclusions

Holmium laser enucleation of the prostate has proven to be an effective and safe surgical treatment, even during the learning curve. The turning point of the procedure depends on proper mentoring and the appropriate selection of cases during the training process.

Introducción y objetivos

El crecimiento prostático benigno tiene una prevalencia de aproximadamente el 60% en hombres de 60 años. La enucleación en bloque y sus variantes, surgida en 2010, son un método para enuclear los tres lóbulos de la cápsula prostática completamente, manteniendo el plano capsular y el esfínter uretral mientras se extirpa la próstata. El objetivo del presente estudio radica en analizar la implementación de la técnica quirúrgica, considerando la curva de aprendizaje y la experiencia acumulada en un grupo de pacientes a priori y a posteriori del desarrollo de la misma.

Material y métodos

Estudio retrospectivo entre julio del 2022 a noviembre del 2024, en el cual se incluyeron a 136 pacientes con sintomatología del tracto urinario inferior por crecimiento prostático con indicación para tratamiento quirúrgico. El sangrado, tiempo quirúrgico, estancia hospitalaria, volumen prostático enucleado así como las eficiencias de enucleación, morcelación, tiempo y energía del láser fueron analizadas.

Resultados

La eficacia del procedimiento fue mayor en el grupo poscurva con una p < 0.0001 comparado con el grupo precurva con un coeficiente de determinación R2 = 0.8091. Las eficiencias de enucleación, tiempo y energía del láser mostraron una diferencia estadisticamente significativa (p < 0.05) a favor del grupo poscurva.

Conclusiones

La enucleación prostática con láser holmium ha demostrado ser un tratamiento quirúrgico eficaz y seguro incluso durante la curva de aprendizaje. El punto de inflexión del procedimiento dependerá de la tutoria y de una adecuada selección de los casos en el proceso de formación.
