Abstract

Universal newborn hearing screening in infants without hearing-risk factors is performed in many settings using otoacoustic emissions (OAE) within the first hours of life. A proportion of initial ‘referrals’ reflect transient, non-pathological factors rather than genuine congenital hearing loss; minimising these is crucial to programme effectiveness.

Objective

To assess the usefulness of repeating OAE screening before hospital discharge in neonates without hearing-risk factors who have an abnormal result on their first test.

Results

A total of 2,667 neonates were studied. On OAE-1, 29.65% had at least one ear with a refer result; of these, 93% underwent repeat testing, with 91% achieving a pass on OAE-2. Overall, 95.46% had a pass by discharge.

Conclusion

The findings and the literature support performing a second hearing-screening test before hospital discharge in infants who did not pass the first. This approach improves detection rates while reducing parental anxiety associated with an abnormal result and decreasing referrals that may contribute to loss to follow-up.

El cribado auditivo universal en neonatos sin factores de riesgo auditivo se realiza en muchos lugares mediante las otoemisiones acústicas (OEA) en las primeras horas de vida. Un porcentaje de recién nacidos que no pasa la primera prueba tienen su origen en factores transitorios que no implican una sordera congénita real. Minimizar estos casos pueden ser determinante para el éxito del programa.

Objetivo

conocer la utilidad de repetir la prueba de cribado con OEA antes del alta hospitalaria en los neonatos sin riesgo auditivo que presenten un resultado alterado en su primera determinación.

Resultados

los neonatos estudiados son 2.667. En la prueba OEA 1 el 29,65% tienen al menos un oído que falla, de estos se repiten un 93%, consiguiendo un 91% de normalidad en la OEA 2. Del total, un 95,46% pasan la prueba al alta.

Conclusión

los resultados y la literatura respaldan la utilidad de realizar una segunda prueba de cribado auditivo antes del alta hospitalaria a los que no pasaron la primera. Dicho enfoque mejora las tasas de detección al tiempo que reduce la ansiedad familiar asociada a un resultado patológico y las derivaciones que pueden implicar pérdidas en el seguimiento.
