Abstract
Objective

This study investigates the effectiveness of intratympanic steroid treatment in managing residual dizziness after benign paroxysmal positional vertigo (BPPV) and its impact on quality of life.

Methods

Fifty-six patients with unilateral posterior canal BPPV without nystagmus after the Epley maneuver were randomized into two groups. Group 1 received intratympanic steroids (two sessions) with the canalith repositioning maneuver, while Group 2 received only the maneuver. Patients were assessed using the Dizziness Handicap Inventory (DHI), Visual Analog Scale (VAS), and residual dizziness evaluation on Day 1 and Day 10.

Results

Baseline assessments showed no significant differences between groups. On Day 10, Group 1 showed significantly greater improvement in residual dizziness (p = 0.036) and Physical DHI scores (p = 0.047), while other DHI and VAS scores remained similar.

Conclusion

This study is the first to show that intratympanic steroids significantly reduce residual dizziness, potentially accelerating recovery and improving quality of life in BPPV patients.

Objetivo

Este estudio investiga la eficacia del tratamiento con esteroides intratimpánicos en el manejo del mareo residual tras el vértigo posicional paroxístico benigno (VPPB) y su impacto en la calidad de vida.

Métodos

Cincuenta y seis pacientes con VPPB unilateral del canal posterior sin nistagmo después de la maniobra de Epley fueron aleatorizados en dos grupos. El Grupo 1 recibió esteroides intratimpánicos (dos sesiones) junto con la maniobra de recolocación canalicular, mientras que el Grupo 2 recibió únicamente la maniobra. Los pacientes fueron evaluados mediante el Dizziness Handicap Inventory (DHI), la Escala Visual Analógica (EVA) y la valoración del mareo residual en el Día 1 y el Día 10.

Resultados

Las evaluaciones iniciales no mostraron diferencias significativas entre los grupos. En el Día 10, el Grupo 1 presentó una mejoría significativamente mayor en el mareo residual (p = 0.036) y en las puntuaciones físicas del DHI (p = 0.047), mientras que otras puntuaciones del DHI y de la EVA se mantuvieron similares.

Conclusión

Este estudio es el primero en demostrar que los esteroides intratimpánicos reducen significativamente el mareo residual, lo que podría acelerar la recuperación y mejorar la calidad de vida de los pacientes con VPPB.
