Abstract

Carotid artery stenosis (CAS), caused by atherosclerotic narrowing of the carotid arteries, has been increasingly recognized as a possible contributor to auditory dysfunction, particularly hearing loss. Several studies have demonstrated an association between CAS and hearing impairment, suggesting that the link is multifactorial and may involve reduced cochlear perfusion, oxidative stress, and vascular–anatomical interactions within the auditory system.

Methods

This systematic review followed the PRISMA 2020 guidelines and was registered in PROSPERO (CRD420251130269). A comprehensive search was performed in PubMed, Scopus, Web of Science, and Google Scholar for peer-reviewed clinical studies, systematic reviews, and experimental research investigating the association between CAS and hearing impairment using audiometry, Doppler ultrasound, angiography, or MRI. The methodological quality of the included studies was assessed using the Joanna Briggs Institute (JBI) critical appraisal tool.

Results

Twenty-six eligible studies met the inclusion criteria. The selected literature comprised clinical and experimental studies evaluating the relationship between CAS and auditory dysfunction. High-frequency sensorineural hearing loss and persistent tinnitus were frequently observed before neurological or cerebrovascular symptoms. Doppler ultrasonography and MRI were consistently used to assess vascular and auditory parameters, improving diagnostic accuracy.

Conclusion

This review supports growing evidence that carotid artery stenosis may represent a potential risk factor for hearing impairment. Audiological symptoms such as hearing loss and tinnitus could serve as early indicators of cerebral ischemia. Routine carotid artery screening is recommended for patients with unexplained auditory complaints. In severe cases, surgical or endovascular revascularization (endarterectomy or stenting) may improve both cerebrovascular outcomes and auditory function.

La estenosis de la arteria carótida (EAC), provocada por el estrechamiento aterosclerótico de las arterias carótidas, se ha reconocido cada vez más como un posible factor contribuyente a la disfunción auditiva, en particular a la pérdida de audición. Diversos estudios han demostrado una asociación entre la EAC y el deterioro auditivo, lo que sugiere una relación multifactorial que puede implicar una perfusión coclear reducida, el estrés oxidativo y las interacciones anatómicas vasculares del sistema auditivo.

Métodos

Se realizó una revisión sistemática conforme a las directrices PRISMA 2020 y registrada en PROSPERO (CRD420251130269). Se efectuó una búsqueda exhaustiva en PubMed, Scopus, Web of Science y Google Scholar para identificar estudios clínicos revisados por pares, revisiones sistemáticas y estudios experimentales que investigaran la asociación entre EAC y la pérdida auditiva mediante audiometría, ecografía Doppler, angiografía o resonancia magnética. La calidad metodológica de los estudios incluidos se evaluó con la herramienta de evaluación crítica del Instituto Joanna Briggs (JBI).

Resultados

Se incluyeron veintiséis estudios que cumplían los criterios de selección. Los síntomas audiológicos tempranos, como la hipoacusia neurosensorial de alta frecuencia y el tinnitus persistente, se observaron con frecuencia antes de las manifestaciones neurológicas o cerebrovasculares. La ecografía Doppler y la resonancia magnética se utilizaron de manera constante para evaluar los parámetros vasculares y auditivos, aumentando la fiabilidad diagnóstica.

Conclusiones

Esta revisión respalda la evidencia creciente de que la estenosis carotídea puede ser un factor de riesgo potencial para la pérdida auditiva. Los síntomas auditivos, como la hipoacusia y el tinnitus, podrían actuar como indicadores tempranos de isquemia cerebral. Se recomienda la evaluación ecográfica carotídea rutinaria en pacientes con síntomas auditivos inexplicables. En los casos graves, la revascularización quirúrgica o endovascular puede mejorar tanto la función cerebrovascular como la auditiva.
