Resumen
Objetivo

Presentar nuestra experiencia en el manejo de osteomas de senos paranasales

Material y métodos

Se realiza una revisión de 14 osteomas frontoetmoidales intervenidos en nuestro hospital entre 1988-2001. Las variables a estudio hacen referencia a la clínica de inicio, localización y tamaño de las lesiones, vías de abordaje empleadas y complicaciones a corto y largo plazo

Resultados

El síntoma referido con más frecuencia fue la cefalea (57%) y la mayoría de los tumores se localizaron a nivel frontal (35%) o frontoetmoidal (35%) con un tamaño medio en el momento de la cirugía de 4,2 cm de diámetro mayor. Todos los pacientes sintomáticos fueron intervenidos empleando técnicas abiertas, salvo dos casos que fueron subsidiarios de Cirugía Endoscópica Nasal (CEN)

Discusión

Aunque los osteomas son tumores benignos, su tendencia al crecimiento progresivo puede provocar complicaciones. Dado que su extirpación no está exenta de riesgos, existe cierta controversia sobre su manejo en pacientes asintomáticos. La CEN reduce la morbilidad del tratamiento pero la localización y el tamaño de las lesiones limitan notablemente el empleo generalizado de la misma

Abstract
Objective

To present our experience in the management of paranasal sinus osteomas

Material and methods

We review 14 patients with frontoethmoidal osteomas removed in our hospital between 1988-2001. The initial symptoms, site and size of the lesions, surgical approaches and short and long term complications were assesed

Results

The most common symptom in our serie was headache (57%); most of the tumors were located in frontal (35%) and frontoethmoidal (35%) regions with a mean size of 4,2 cm (maximum diameter) at the time of surgery. All symptomatic osteomas were resected by classic external surgical approaches but two cases were removed by endonasal surgery

Discussion

Although osteomas are bening tumors, they may enlarge progressively and produce complications. Their resection is not free of risks, so there is some controversy about the management of asymptomatic patients. Endonasal surgery reduces the morbidity of the treatment but the location and size of the lesions do not allow to employ this technique in all cases
