Extraparenchymal renal artery aneurysms. Surgical results and impact hypertension

Resumen
Objective

This report evaluates the role that hypertension plays in patients with extraparenchymal renal artery aneurysms. We also review surgical management and results.

Materials and methods

From January 1978 through December 1999, 19 consecutive patients with 23 extraparenchymal renal artery aneurysms were operated with renal revascularization techniques. Seventeen patients (89,5%) had arterial hypertension.

Results

Twelve of 16 hypertensive patients revealed renovascular hypertension. Twenty of the aneurysms were permeable, one was chronically thrombosed and one patient presented with acute thrombosis of abdominal aortic and bilateral renal aneurysms. Eleven of the 20 permeable cases had significant stenosis in the preoperative arteriography. Seventeen aneurysms (74 %) were located on the main trunk of the renal artery. There were no postoperative exitus. Early patency was 100%. The long-term rate-of cure/ improvement of hypertension was 65% (91% of these patients had revealed renovascular hypertension and renal artery stenosis). Cumulative patency was 89.2%.

Conclusions

Our basic surgical indications for extraparenchymal renal artery aneurysms are renovascular hypertension, preservation of renal function and prevention of rupture. Surgical treatment offers good results of long-term patency rate.
