Comparison between dynamic popliteal phlebography and color Doppler ultrasonography in the diagnosis of venous insufficiency in the popliteal fossa

Resumen
Objectives. To compare the results obtained using dynamic popliteal phlebography (DPP) and color Doppler ultrasonography (CDU) in the diagnosis of venous insufficiency in the popliteal fossa. The different techniques employed for the study of venous disorders occurring in this region are reviewed.
Patients and methods. We studied 62 limbs presenting venous insufficiency involving the popliteal fossa in 44 patients (33 women and 11 men, with a mean age of 49.8 years). The existence of reflux at this site was confirmed by continuous-wave Doppler in every case. The patients underwent CDU first, followed on a different day by DPP.
Results. CDU disclosed the presence of external saphenous vein reflux in 23 cases, calf vein reflux in 32 limbs, deep venous system reflux in 13 and reflux of the perforating veins of the popliteal fossa in 4; none of the limbs presented Giacomini's vein. According to DPP, 21 limbs presented external saphenous vein reflux, 34 cases showed calf vein reflux, 11 limbs presented deep venous system reflux and there were 3 cases of reflux of the perforating veins of the popliteal fossa; Giacomini's vein was detected in 3 limbs. There were no significant differences between the two methods.
Conclusions.. In expert hands, CDU correctly maps the different types of reflux present in the popliteal fossa. Thus, this method is a valid substitute for phlebography, which would still be indicated in cases that are difficult to interpret or involving anatomic abnormalities or recurrences.
