Resumen

A medida que se ha ido reduciendo el tiempo de ingreso hospitalario en pacientes con traumatismo cráneo-encefálico, los programas de rehabilitación post-aguda han ido incrementado su importancia, existiendo múltiples investigaciones recientes que evalúan la efectividad de estos programas. En general estos estudios demuestran una mejoría en independencia funcional, productividad y función social. En esta fase la intervención terapéutica se orientará hacia el mantenimiento y/o mejoría del nivel funcional conseguido y la detección de posibles complicaciones y su tratamiento.

Después de un traumatismo craneoencefálico pueden aparecer una serie de complicaciones a consecuencia de la lesión cerebral, algunas de ellas a su vez, podrán manifestarse en una fase más avanzada, como son: la hidrocefalia, la epilepsia postraumática, disfunción endocrina, osteomas, alteraciones en el estado nutricional y la función respiratoria, así como repercusión de las alteraciones neuropsicológicas en esta fase evolutiva.

Por consiguiente, se aconseja la práctica de revisiones periódicas protocolizadas en las que de forma periódica se realice una exploración clínica, exploración neurológica, exploraciones complementarias, valoración funcional y social. Ello permite detectar posibles complicaciones que tendrán o no, tratamiento en ese momento, así como situaciones que puedan requerir la intervención de otros especialistas.

Summary

As hospital admission time has been decreasing in patients with cranioencephalic traumatism (CET), post-acute rehabilitation programs have been increasing in importance, there being many recent investigations that assess the effectiveness of these programs. In general these studies show an improvement in functional independence, productivity and social function. In this phase, the therapeutic intervention will be oriented towards maintenance and/or improvement of the functional level achieved and the detection of possible complications and their treatment.

After a CET, a series of complications can appear due to the brain lesion, some of them, in turn, can be manifested in a more advanced phase, such as: hydrocephaly, post-traumatic epilepsy, endocrine dysfunction, osteomas, alterations in the nutritional stage and respiratory function, as well as repercussion of the neuropsychological disorders in this evolutive phase.

Consequently, performing periodic protocolized reviews in which a clinical examination, neurological examination, complementary examination, functional and social assessment are performed is recommended. This makes it possible to detect possible complications that may or may not have treatment at this moment as well as situations that can require the intervention of other specialists.
